Transplant care doesn’t end when a patient leaves the hospital.

At IV Solutions RX, our Transplant Program is designed to support transplant recipients and their healthcare teams as patients transition from the transplant center to ongoing outpatient care.

We understand that transplant patients can have complex clinical needs requiring careful coordination among transplant physicians, specialists, pharmacists, infusion nurses, insurance providers, caregivers, and patients.

Our goal is simple: Help make that journey easier to navigate.

IV Solutions RX works collaboratively with transplant centers, physicians, advanced practice providers, transplant coordinators, pharmacists, case managers, discharge planners, and other members of the patient’s healthcare team.

From referral and benefits investigation through therapy coordination, medication delivery, nursing services, and ongoing communication, our team helps support continuity of care throughout the patient’s treatment journey.

Our Transplant Program can assist with:

Referral Coordination
We work with the referring transplant team to collect the clinical documentation, prescriptions, laboratory information, and other information required to evaluate and process the referral.

Benefits Investigation & Prior Authorization Support
Our team helps navigate insurance requirements and works with providers to obtain the documentation necessary to support authorization for prescribed therapies.

Specialty Pharmacy Services
Our pharmacists and clinical team review each referral based on the patient’s prescribed therapy, clinical condition, physician orders, available medical documentation, and applicable payer requirements.

Specialty Infusion Therapy
For eligible patients and prescribed therapies, IV Solutions RX coordinates specialty infusion services in the appropriate site of care.

Experienced Infusion Nursing
Skilled infusion nurses can provide medication administration, patient assessment and monitoring, education, vascular-access support when applicable, and post-infusion reporting.

Medication, Supply & Equipment Coordination
When applicable, prescribed medication and necessary infusion supplies and equipment are coordinated for delivery to support timely therapy.

Patient & Caregiver Education
We help patients and caregivers understand their prescribed therapy, administration process, potential side effects, treatment schedule, and the importance of communicating changes or concerns to their healthcare team.

Ongoing Communication
Our team helps maintain communication among the patient, prescriber, transplant team, pharmacy, nursing professionals, and other involved healthcare providers.

Our clinical team understands that every transplant recipient is different. We work with healthcare providers caring for patients across transplant populations, including:

  • Solid organ transplant
  • Kidney transplant
  • Liver transplant
  • Heart transplant
  • Lung transplant
  • Pancreas transplant
  • Multi-organ transplant
  • Hematopoietic stem cell and bone marrow transplant patients when therapies fall within our clinical and pharmacy capabilities.

Services and therapy availability are dependent upon the patient’s individual clinical needs, prescription, payer requirements, pharmacy licensure, nursing availability, manufacturer requirements, and other applicable criteria.

Specialty Infusion Support After Transplant

Transplant recipients may develop complications or secondary conditions that require specialty pharmaceutical or infusion support.

    • Antibody-Mediated Rejection (AMR)
    • Acute or chronic rejection
    • Donor-Specific Antibodies (DSA)
    • Transplant-related hypogammaglobulinemia
    • Immune-mediated complications
    • Post-transplant infections
    • Desensitization and antibody-reduction protocols
    • Other transplant-related conditions requiring specialty medications or infusion therapy

Depending upon the patient’s diagnosis, physician orders, payer requirements, and applicable clinical criteria, IV Solutions RX can evaluate referrals for therapies within our specialty pharmacy and infusion portfolio.

Our capabilities include IVIg and SCIg therapy, along with a broader portfolio of specialty medications and infusion services.

Therapies may include, not limited to:
IVIG • Rituximab (Rituxan®/biosimilars) • Belatacept (Nulojix®) • Infliximab (Remicade®/biosimilars) • Eculizumab (Soliris®) • IV corticosteroids • IV antivirals • IV antibiotics/anti-infectives • Other specialty biologic and immunomodulatory therapies

We also support access and coordination for commonly prescribed transplant medications, including, not limited to:
Tacrolimus (Prograf®/Envarsus XR®) • Mycophenolate mofetil (CellCept®) • Mycophenolic acid (Myfortic®) • Cyclosporine (Neoral®/Gengraf®) • Sirolimus (Rapamune®) • Everolimus (Zortress®) • Prednisone • Valganciclovir (Valcyte®) • Antifungal and other prophylactic therapies

Rather than approaching the patient as simply another prescription, our team evaluates the complete referral and works with the treating healthcare providers to help coordinate the services required for therapy.

IVIg & Immune Globulin Support

Immune globulin therapy can be an important component of care for appropriately selected patients across a variety of disease states and clinical circumstances.

IV Solutions RX offers a broad portfolio of IVIg and SCIg products, subject to prescription, clinical appropriateness, payer authorization, product availability, and applicable requirements.

Our team can help coordinate:

  • Clinical referral review
  • Benefits investigation
  • Prior authorization support
  • Required clinical documentation
  • IVIg/SCIg medication coordination
  • Infusion nursing
  • Infusion supplies and equipment
  • Laboratory coordination when ordered
  • Patient and caregiver education
  • Ongoing therapy communication
  • Refill and delivery coordination

All therapy is provided pursuant to the treating provider’s orders and applicable clinical, payer, regulatory, and pharmacy requirements.

Transitioning From Hospital to Home

The transition following transplantation can be complicated.

Patients and caregivers may suddenly find themselves coordinating medications, laboratory testing, follow-up appointments, insurance requirements, specialty therapies, infusion schedules, and multiple healthcare providers.

IV Solutions RX helps simplify the specialty pharmacy and infusion portion of that transition.

Our team works with transplant centers, hospital discharge teams, case managers, physicians, patients, caregivers, payers, and nursing professionals to help coordinate prescribed specialty therapies as patients move into outpatient care.

One Referral. One Coordinated Team.

Our approach is built around communication, accessibility, and coordination.

Provider → Referral → Clinical Review → Benefits Investigation/Authorization → Pharmacy Coordination → Nursing Coordination → Medication & Supply Delivery → Therapy → Follow-Up

Throughout the process, our goal is to keep the patient and referring healthcare team informed.
